The rebate claim form must be filled out in its entirety and  mailed with the original UPC barcode (cut from the product box) and purchase receipt (a copy of the receipt is acceptable) to the address printed on the rebate form. The device must be a qualifying product purchased from a participating authorized reseller within specified purchase dates printed on the rebate form.   The rebate claim must be also postmarked within the specified redemption period.
